Measure Title: Appropriate Dosing of New Oral Anticoagulants (NOACs) in Patients with Renal Dysfunction

Eligible Patients: Adults with a current prescription for NOAC who have a serum creatinine lab result recorded in the past year (excluding patients with edoxaban prescription)

Criteria that Meets Measure: Patients on appropriate daily dose according to renal function

Apixaban:
If SCr < 1.5 mg/dL AND age >= 80 yrs AND weight <= 60 kg, max daily dose 5 mg
If SCr > 1.5 mg/dL AND (age >=80 yrs OR weight <=60 kg), max daily dose 5 mg
For all others, max daily dose 20 mg
Dabigatran:
If CrC > 30 ml/min, max daily dose 300 mg
If CrCl 15-30 ml/min, max daily dose 150 mg
If CrCl < 15 ml/min, avoid use
Rivaroxaban:
If CrCl > 50 ml/min, max daily dose 30 mg
If CrCl 15-50 ml/min, max daily dose 15 mg
If CrCl < 15 ml/min, avoid use


Calculating the Daily Dose

The Dose Per Day calculation is:

Single-Dose-Amount * Doses-Per-Day * Package-Strength


For example, if the dose is 2 tablets three times a day, and each tablet is 50MG, the Daily Dose amount would be
2 * 3 * 50MG = 300MG
