| Denominator | Patients 18 years of age and older who were diagnosed with acute myocardial infarction (AMI), coronary artery bypass graft (CABG) or percutaneous coronary interventions (PCI) in 24 to 12 months prior, OR who had an active diagnosis of ischemic vascular disease (IVD) in 12 months. | Numerator | Most recent LDL result < 100 - (last result documented in 12 months). |
| Exclusion/Exception | Patients with hospice care in 12 months. |
