During the Data Validation stage and whenever an issue with a measure or patient list is found, Clinigence support will need the assistance of the practice to find the origin of the problem and resolve the issue.


There are several types of problems and each one has typical origins.


The measure performance numbers are too low (or too high)

This patient should be in the numerator, but is not. Pt ID 10045509, Sally Jones DOB 12/4/1964 had a mammogram on 12/15/2014. See attached screenshot.
Items are mapped incorrectly

Find 1-2 example patients who you believe has the required data items and create a helpdesk ticket with the following:

For example:

I think the mammogram numbers are too high. Here's the URL: https://solution.clinigence.com/Practices/4924110/Programs/119/Guidelines/837

This patient is in the numerator, but should not be. Pt ID 10045509, Sally Jones DOB 12/4/1964 had a mammogram ordered on 12/15/2014 but it was never completed.  It is showing in Clinigence as performed on 12/15/2014.

Configuration issues

Provide example patients who are either missing from the denominator or numerator or are missing from the denominator or numerator. Include the relevant information as in the examples above.

Select 1-2 example patients and create a helpdesk ticket with the following:

  • The URL of the measure in question. (It should look like this: https://solution.clinigence.com/Practices/4924110/Programs/119/Guidelines/849/Measures/1369)
  • The details of the example patient: Name, DOB, date of the treatment in the patient's chart -- if the treatment was documented on a different date than the treatment was performed be sure to note that information. Screen captures are helpful here, but be sure that it includes all the needed details and is legible.
