The purpose of the validation worksheet is to ensure that the numbers for reporting are as accurate as possible. It requires someone at the practice or VAR to access the EMR directly to compare the data from the Clinigence performance dashboard with a small sample of patient charts, so someone who is very familiar with the EMR and with the patient population is preferable.

Instructions for Validation Worksheet:

For each measure, review a small percentage of patients (at least 4-5 patients) in both the numerator and complement (Outside Target Criteria). For the numerator patients, confirm that the required treatment is documented in the patient’s chart and that the date presented in Clinigence is the same as in the EMR. For the complement patients, confirm that the required treatment for the numerator is missing or that the item is expired – if a date is displayed in Clinigence confirm that is the most recent date for the item in question.

Please document in the worksheet the patients that were validated. The Clinigence patient ID is sufficient. If there is a discrepancy, please include the following:

1) Is patient in the wrong class (numerator/complement)?

2) Is the date shown in Clinigence correct? --If not, provide the correct date.

3) If the patient should be in the numerator, but is not provide the details for the item(s) that would qualify for the numerator. For example, if the numerator criteria requires a Depression Screening within 12 months and that information is not correct in Clinigence, provide a screenshot showing the wording used for this item and date of most recent order.
